Epic Bridges/Data Aggregation and Pop Health Project Manager

Responsibilities:

• Help complete our Data Aggregation Project for Population Health
• We really are looking for someone with experience/knowledge in converting outside data from other EMR’s and interfacing into EPIC- so an emphasis will on having that knowledge, as well as good Project Management skills
• Collaborate on plan development, communication plans and budgets
• Monitors project scope and drives communication to support scope control
• Manages project issues and risks
• Participates in capturing business requirements, architecture reviews, etc.
• Maintains Service Now project record and Sharepoint sites
• Review invoices for accuracy before processing
• Follows IT-PMO standards and best practices
• Lead and facilitate Leadership Meetings
• Create/hold accountable milestones and tasks necessary for Data Aggregation
• Work with other analysts and consultants to ensure work is being completed in a timely manner
• Ensure partner sites are delivering data, establishing connectivity per project requirements
• Provide status updates, address and escalate issues as needed
• Ensure project is on time and budget
